Pre-court information form - personal safety intervention order matter (PSIO)

If you have an upcoming court hearing date you must contact the court about your matter at least seven days before your court hearing. You can do this by completing this form.

You must complete the form if you are the affected person or the respondent to the intervention order application.

This form provides the court with the following information:

• your address and contact details
• if you would like to speak to an LGBTIQ+ practitioner, and
• how you would like your matter to proceed.
