Pre-Court Information Form - Family Violence Intervention Order matter (applicant and affected family member)

If you are the applicant or affected family member, and you have an upcoming court date, you need to contact the court about your matter at least seven days before your court hearing. You can do this by completing this form.

You must complete the form if you are the applicant or the respondent to the intervention order application.

This form provides the court with the following information:

• your address and contact details
• if you would like to speak to a lawyer or family violence practitioner, and
• how you would like your matter to proceed.
