This form should be used if a federal jurisdiction application to the Magistrates’ Court is being made, and the matter relates to a section of the following Acts:

• Equal Opportunity Act 2010
• Health Records Act 2001
• Privacy and Data Protection Act 2014
• Racial and Religious Tolerance Act 2001 
• Gender Equality Act 2020
• other, as specified.

This form must accompany the Form 10A and filed together with relevant supporting documentation.
